Pre-participation Athletic Forms
To participate in intercollegiate practice at Sierra College, please ensure that all required forms are completed and on file in the athletic training room. This includes setting up or updating your account on the SportsWareOnline Injury Tracking Software and completing a pre-participation physical evaluation (PPE).
